that once you have completed this form, we will need to contact your previous veterinary practice to request your pet’s clinical history. Once this has been received, we will contact you to arrange an appointment. We will be unable to book an appointment for your pet until we have received this medical history.
If this is your pet’s first visit to a veterinary practice and there is no previous medical history to request, we will contact you by telephone to arrange an appointment and confirm your registration with us.
